This lady has a high BMI and struggles with the rubbing of her breasts and the irritation and inflammation this creates. The excess breast weight has led to increased neck and shoulder pain. When nipples are very low and there is a significant amount of tissue to remove, it is often too risky to try and keep the nipple attached to its blood supply. This lady has had a breast reduction technique using a free nipple graft. The mole on this lady’s right breast shows how far the nipple has been lifted. The new tan lines also reflect how the breast footplate is not a lot higher. Although traditionally the free nipple graft technique is not believed to be as superior aesthetically it may be a more reliable technique for women at high risk of nipple areola necrosis and in complex breast reduction cases.

In this scenario, the main issue was related to the functional issues associated with excess weight and breast implants and fat transfer were not a priority.
