This lady wished to improve the appearance of her periorbital region. She has a lot of excess skin in the upper lids with hooding but it is in her lower lids where her ageing is even more apparent. The orbital septum has weakened and the orbicularis oculi muscle no longer contains the fat in the eye and the septum. The midface has dropped and this has given the appearance of aging at the lid cheek junction as seen preoperatively. She also has a superficial burn on her nose in the preop picture.

This lady underwent an upper lid blepharoplasty and this has improved the hooding and skin excess in the upper lids improving the lid show. In the lower lids the fat within the septum has been repositioned and the arcus marginalis and orbicularis retaining ligaments have been released with elevation of the midface and restructuring the orbicularis and midface onto the inside of the orbital rim through the incision used for the upper lid blepharoplasty. The excess skin has been removed. This technique perceptually raises the midface and decreases the distance from the pupil to the cheek. This in turn improves the appearance of ageing.

The results are shown at 6 months and these have led to an improvement in the aesthetic appearance of the periorbital region.
